If you would like to apply for online access to any or all of the following:

  • Your summary care record
  • Your repeat prescriptions
  • Appointment booking
  • Your record of immunisations

Please complete the following three steps in full:

  • Complete the application form on this page.
  • Upload an official letter, e.g. bank statement or utility bill, showing proof of address (dated in the last three months)
  • Upload either: your passport (open at the ID page), OR your photo driver’s licence. The ID document must be clearly legible

Alternatively, you can come to the surgery with the above two forms of ID and complete the application form in person.

If you would like to apply for access to your full medical record, you will need to come to the surgery with the above two forms of ID and complete the application form in person. Please note that setting up access to your full detailed medical record will take us longer to process than the other services – (please allow up to 21 days).
